By 30 January 2004 all interested teams should have returned the application forms to Ramblers, who are once again organising this popular event.The application form for the Top Score 7-A-Side Soccer Tournament for the year 2004 can be collected at the Ramblers Club House in Pionierspark, Egoli Fruit & Vegetable, as well as Namib Mills in the Northern Industrial Area.

Please take the following important information into account when completing the application form:

1. only proper black & white passport photos are accepted;

2. as further correspondence will be conveyed to the participating teams via e-mail, including a weekly update, the correct e-mail address of your firm is required;

3. the full name of the person signing the application form should be clearly written in printed letters;
